Hospital Links Program

Hospital Links program is one of the five services within the Community Health Division which in turn is part of the Primary and Population Health Directorate and is a partnership program with UnitingCare Wesley Adelaide funded by the Department of Health. The program works in collaboration with relevant community services, housing, income support, welfare, justice and health agencies to respond to the many needs of homeless youth and improve their health outcomes.

Young people can be referred to the program if they do not have access to safe, secure and adequate housing or are already housed by the Supported Accommodation Assistance Program (SAAP). Referrals are accepted from workers in both government and non government agencies and directly from young people if they are on an independent income.

Services provided

The program provides a specialist assessment service with interim case management provided for those young people with no community supports and can assist hospital staff with discharge planning.

The service is provided by experienced social work, midwifery, clinical general and mental health nursing staff in a flexible, mobile and holistic manner

Referral Criteria

  • Referrals must be voluntary and with informed consent
  • The young person needs to be homeless
  • The young person is aged between 12-18 years
  • The young person is aged between 12-24 years if they are pregnant/parenting

The Community Clinical Nurse/Midwife (mobile 0417 819 397) at Women's Health Statewide, 64 Pennington Terrace North Adelaide 5006

Referrals can be made directly to the relevant Hospital Links worker whose contact details and availability can be obtained from the Women's and Children's Hospital switchboard on 8161 7000 or via e-mail cywhs.hospitallinks@cywhs.sa.gov.au.
