Sleep Disorder Unit

The outline should be a brief summary of the department/service. Its purpose is to allow the reader to decide whether this is the page they are looking for.

The outline should be 200 words or less, and written in several paragraphs to make reading easier. Dot points can be used to help reduce the density of the outline. To expand the amount of information use links rather than ‘crowding’ the outline section.

How to access the service

Include here relevant information about accessing the service eg

Information about referrals (if applicable)

  • Whether referrals are needed
  • Who can give referrals
  • Paperwork that needs to be completed by client/referring health professional

How to make an appointment etc

Information for Families

The Information for Families section should include any additional information for parents/families/patients regarding the service. The information for families topics should be presented as a list of links. There will be a general section about Hospital services. Only include general things that are very pertinent to the Service/Department.

Some examples are:

  • How to determine whether you/your child needs the service
  • What to bring to the appointment
  • What kinds of tests/examinations might occur
  • What kinds of things parents might tell their children to prepare them for appointments/assessments/treatments etc
  • How to organise an interpreter (if required)
  • What other services/support groups are available
  • Aspects of the treatment that may affect other members of the family ie overnight stays, assessments/treatments at other locations/visiting times/accommodation etc.
  • What it will mean for your child – missing school etc
  • Other services provided by the hospital that are offered eg counselling

The forms section should contain a list of forms/other paperwork that can be downloaded, and completed at home. Only forms that are up-to-date and relevant should be included.

We envisage resources as being more directed toward health professionals – though this should not preclude patients and families. The resources should be presented as a list of links.

Some examples of resources are:

Procedural guidelines
Codes of ethics
Information for service providers
Philosophical frameworks
Evaluation of service procedures
Health care models used
Business plans

Research Activity

Include here brief details of any current/recent research projects if appropriate - if not delete heading.  To avoid crowding use hyperlinks

Details of wards if applicable.  Delete heading if no wards

The days should be written first and the times second. The days and times should fall into columns, to make them easy to read.
