Complex Care Coordination Unit

Complex Care Coordination Unit has been established as part of the organisational redesign, is a network wide service with the aim to work in partnership with WCHN staff and consumers to assist in Strategic Region wide service improvement and redesign whilst also improving the patient and family’s journey.
